Research Survey for Teachers of Students who are Deaf/Hard of Hearing and Educational Audiologists

“Samantha Gustafson, AuD, PhD and Nicole Pilling, MED, AVEd, LSLS cert. at the University of Utah would like to invite you to participate in a research study examining current collaborative practices between educational audiologists and teachers of students who are deaf and hard of hearing (TSDHH). For this study, we are interested in describing current collaborative practices and identifying barriers and facilitators of collaboration between educational audiologists and TSDHH.

To participate in this survey, you must be a licensed audiologist who works at least part time in an education setting OR a licensed teacher of the deaf. Your participation in this research project is completely voluntary. There are no known risks to participation beyond those encountered in everyday life. Your responses will remain confidential and anonymous. No one other than the researchers will know your individual answers to this questionnaire. If you agree to participate in this study, please answer the questions as best you can. By completing the survey, you are providing your informed consent to participate in research. The survey should take approximately 10 – 15 minutes to complete.
